Preparticipation Physical Evaluation

PISD requires that every athlete complete the medical history portion of The Medical History Form (page 1) and turn it in to the coach every year that you participate in PISD athletics. A physical exam and doctor’s signature (page 2) must be completed when entering grades 7, 9 and 11. A physical exam and doctor’s signature may be required in additional years if you answer “yes” to specific questions on the Medical History portion of the form. You will be notified if a supplemental physical exam is required for your athlete. Your athlete cannot begin to participate in your sport or related activities (such as weight training) until the appropriate forms are on file with your school. If you play multiple sports, only one form is required.
